Customer Forms
If you're a new customer to Unity please Request an Information Pack to apply for information on opening an account with online banking.
Existing customers can register for Internet Banking and Telephone Banking by printing and completing the following application forms:

3. Change of Signatory Request form - this form is to notify us of changes to signatories on your account. You can notify us of a change by printing and completing this form. Alternatively, you can complete the form on-line and print it out by clicking here.
Please ensure that all new signatories read the three documents below ('Terms and Conditions 2008', 'Information on proving your identity' and 'Customer Information & how we use it') before completing the form. When completing your request, you should complete details of any NEW signatories being added to the acount. Any existing signatories remaining on the account should also complete their details in full. Please also remember to complete the resolution and specimen signature details.
Once completed, please return the form to FREEPOST, Unity Trust Bank, Nine Brindleyplace, Birmingham B1 2BR. You do not need to use a stamp.

4. Details of Signatories form (where appropriate) - this form should only be completed by any signatory who is not going to become a telephone banking user and who has not previously been required to prove their identity to Unity Trust Bank.
Please circulate the 'Terms and Conditions 2008' booklet and 'Information on proving your identity' document to each person who applies to become an authorised user.
5. Transfer of Funds Overseas form - if you need to make a foreign transfer, please download this form. All sections of the form should be completed and it should be sent to us signed in accordance with your mandate. Remember to retain a copy of both the front and back of the transfer request for your records as these contain terms and conditions which are in addition to our standard terms and conditions.
Please note that we are only able to accept original signed copies of transfer request forms. Details of the charges for making a foreign transfer can be found in our 'Standard Service Tariff' leaflet, which can be found here.
