Existing customers can register for telephone banking by printing and completing the attached forms:-

2. Details of Signatories Form (where appropriate) - this form should only be completed by any signatory who is not going to become a telephone banking user and who has not previously been required to prove their identity to Unity Trust Bank.

Please circulate the 'Proving your identity' information and 'Terms & Conditions' to each person that applies to become an authorised user.
